Picture a disease that quietly steals the center of someone's vision, injection by injection, year after year. That's the reality for millions living with abnormal vessel growth beneath the retina, and it's why the choroidal neovascularization market has become one of the more closely watched corners of ophthalmology. What was once a story about managing a chronic condition is turning into a story about curing it outright, as gene therapies edge closer to reality alongside the anti-VEGF drugs that have dominated treatment for over a decade.

What's Actually Happening to the Choroidal Layer

Tucked beneath the retina, the choroidal tissue is normally a quiet supporting player in vision. But when disrupted, it can sprout fragile new blood vessels that leak fluid or blood, blurring or distorting whatever a person looks at. The usual triggers are wet age-related macular degeneration, high myopia, or inflammatory eye disease. In the US, myopia-driven cases make up a small slice of the population, yet they carry an outsized impact on quality of life. Germany tells a similar story, where 1–3% of people live with pathological myopia, and a meaningful share of them go on to develop this vascular complication. Behind it all sits a surge of vascular endothelial growth factor, spurred by low oxygen and cellular stress, which pushes the eye into building vessels it was never meant to have. Doctors catch it early using optical coherence tomography and fluorescein angiography — imaging that reveals fluid pooling under the retina before permanent damage sets in.

Inside the Choroidal Neovascular Membrane

The real culprit doing the damage is what's called a choroidal neovascular membrane — a tangle of abnormal vessels that pushes through Bruch's membrane and invades the retinal pigment epithelium. It's messy: bleeding, scarring, and leakage all follow, and if nobody intervenes, the vision loss becomes permanent. The numbers paint a sobering picture. Close to 1.5 million Americans live with wet or advanced age-related macular degeneration, while Germany counts roughly 3.27 million affected individuals. In France, prevalence sits around 1.06%, climbing steeply with age and appearing far more often in women. Japan's pattern looks different again, with high myopia driving a disproportionate share of new cases — a reminder that this isn't a one-size-fits-all disease, and regional screening strategies need to reflect that.

The Anti-VEGF MOA Still Running the Show

Right now, treatment still hinges on the anti VEGF MOA — blocking the very growth factor that fuels this whole process. Genentech's VABYSMO (faricimab-svoa) broke new ground as the first bispecific antibody in ophthalmology, hitting both angiopoietin-2 and VEGF-A at once to keep vessels stable for longer stretches between visits. Novartis countered with BEOVU (brolucizumab-dbll), a smaller antibody fragment built to penetrate deeper into retinal tissue and hold disease activity in check. Both have genuinely moved the needle on vision outcomes. But patients still feel the weight of frequent injections, and response varies enough from person to person that the search for something better hasn't slowed down — laser treatment and photodynamic therapy still get used in select, trickier cases.

Where the Retinal Neovascularization Market Is Headed Next

This is where things get genuinely exciting. The wider retinal neovascularization market is on the cusp of a shift from repeat treatment to one-and-done therapy. AbbVie and REGENXBIO's surabgene lomparvovec (ABBV-RGX-314) delivers a gene straight into retinal cells using a viral vector, aiming for continuous VEGF suppression without repeat injections — pivotal trial results are expected in 2026. Neuracle Genetics' NG101 recently earned FDA Fast Track status and wrapped its first dosing cohort. Meanwhile, Adverum Biotechnologies' Ixoberogene soroparvovec has pushed into a pivotal Phase III trial, with enrollment wrapping in late 2025 and results due in early 2027. If even one of these clears the finish line, it could fundamentally change what a diagnosis means for patients.

The Bigger Picture

Across the US, EU4, UK, and Japan, the same forces keep pushing this market forward: an aging population, sharper diagnostic tools, and a genuine pipeline of therapies that could finally ease the injection treadmill. The next decade looks less like incremental improvement and more like a turning point — one where lasting vision preservation stops being aspirational and starts becoming standard.
